Participant Website Survey

To the participants in this study,

You are being asked to participate in a research study.  Before you give your consent to be a volunteer it is important that you read the following information to be sure you understand what you will be asked to do.

The following survey is one part of an evaluation of the Orientation to Ontario (O2O) Pilot Project.  O2O is bilingual initiative co-funded by Citizenship and Immigration Canada (CIC) and the Ontario Ministry of Citizenship and Immigration (MCI). It is a consultation to get the input of the settlement sector and other stakeholders in order to determine best practices and make recommendations on service delivery models.

This survey will be distributed to all participants using the website.   In addition to this survey, facilitators and the project team will also complete a survey to offer their opinions as well.  Your participation in this survey will help us improve the project for newcomers looking for standardized settlement information and access to community services in Ontario.  In order to proceed, we need your consent.

The survey will take about 10-15 minutes complete.  The survey will ask about your opinions of the website and suggestions for improvement.  At the end of the survey you will have the option to enter your name for a chance to win a prize of $100.00.

This evaluation is being conducted by an independent evaluator PINPOINT RESEARCH AND CONSULTING SERVICES, INC.  Your responses will be automatically saved and the data will be stored on a server.  All raw data will only be accessed by the PinPoint Research team.  All information will be kept in strict confidence and stored in a locked cabinet at PINPOINT’s office.  All electronic data will be stored on a password protected computer.   All information will be reported in such a way that individual persons, workshop locations and facilitators cannot be identified.  All raw data will be destroyed three years after the completion of the study.

Your participation is completely voluntary.  If you decide to participate, you are free to withdraw your consent, not answer any question you are not comfortable with or end the survey at any time without consequence, penalty or judgement.   Finally, you are free to ask any questions about the research and your involvement with it.

If you have any complaints, concerns or questions, please email 020@Costi.org or feel free to contact Michelle Goldberg, at (905)707-7732 or michelle.goldberg@utoronto.ca.
