PWA Accessibility Survey
The Toronto People With AIDS Foundation (PWA) is conducting a survey of people living with HIV/AIDS in Toronto to determine what we can do to make HIV health and treatment information more accessible. We would appreciate your time in participating in this survey, as your feedback is important to us and helps us to improve our services. If you need help completing the survey, please don’t hesitate to contact us.
This survey should only take 10-15 minutes of your time and is completely confidential.
This survey is composed of two parts. The first part has questions related to how you access HIV-related health and treatment information and any barriers you may experience. The second part collects demographic information. We collect demographic information so that we can better understand who is answering this survey (on a population level) and what their needs are. It also helps us to identify if there is a population that is not getting our survey, which itself can indicate a barrier to accessing information. The information you provide will not identify you as an individual in any way.
