CONTEST RULES 1. TO ENTER complete and submit the online form which is located at http://fluidsurveys.com/surveys/teluscsr/telus-csr-report-contest/. Only one online entry per person will be accepted for the duration of the contest. In the event of a dispute, the person who is the holder of the email address specified on the online form will be deemed to be the entrant.]\ Submit your entry form on or before 5:00 pm PST, September 7, 2012 ("Contest Closing Date"). Contest runs from 8:00 am PST, September 3,. 2012 to 5:00 pm PST, September 7, 2012. No purchase necessary. TELUS does not accept entries for this contest by phone, fax, courier, mail or email. This contest is intended for residents of Canada only and will be interpreted according to Canadian law. 2. THE PRIZE: The awarded prize(s) will consist of: 3 prizes each consisting of 1500 Bravo points (Approx. retail value $150.00 each) Prize(s) must be accepted as offered with no substitutions. Non-cash Prizes are non–transferable and not redeemable for cash. TELUS reserves the right to substitute a Prize which is, in their sole opinion, of equal or greater value, in the event that any Prize (or any part thereof) becomes unavailable for any reason whatsoever, or to cancel this promotion without notice. The odds of winning a Prize are dependent upon the total number of eligible entries received. 3. THE DRAW: A random draw for the Prize will be made by a contest administrator on September 12, 2012 in Burnaby at approximately 12:00 pm PST from all eligible entries received on or before the Contest Closing Date. In order to win, the selected entrant must have first correctly answered unaided the skill-testing question on the entry form, and will be required to sign a standard release form. In the event that an entrant whose ballot is drawn is not eligible, the contest administrators shall have the right to redraw another entrant until the Prize is awarded. Failure to comply with the Contest Rules or to sign and return the release form by 5:00 pm PST, September 17, 2012 will result in disqualification and another entry will be drawn. The selected entrant(s) will be notified by telephone and by email. No correspondence will be undertaken, except with the selected entrant(s) By accepting or participating in a Prize, the winner consents to the use of their name, address, voice, statements relating to the Contest or Contest Sponsor, and photographs or other likenesses, without compensation, in any publicity carried out by the Contest Sponsor or any related entities in connection with this contest. 4. THE CONTEST IS OPEN to all Canadian employees of TELUS Corporation or any of its subsidiaries. Entrants must be employed both at the time of registration and at the time of the award and must have reached the age of majority in their province of residence in order to be eligible to enter the contest and obtain a prize. The contest is not open to any consultants or contractors retained by TELUS or any of its subsidiary companies. Entrants must be employees of TELUS at the time of entry and/or at the time of prize award and must be in good standing (not on notice or subject to any disciplinary action) in order to be eligible to win the prize. 5. ALL ENTRIES become the property of TELUS, who assume no responsibility for lost, delayed, destroyed or misdirected entries and no correspondence will be entered into except with selected entrants. TELUS assumes no responsibility for lost, stolen, delayed, damaged or misdirected entries or for any failure of any website, for any problems or technical malfunction of any computer online systems, servers, access providers, computer equipment, software, failure of any e-mail or entry to be received by TELUS on account of technical problems or traffic congestion on the Internet or at any website, or any combination thereof including any injury or damage to an entrant's or any other person's computer related to or resulting from this contest. TELUS reserves the right, at its sole discretion, to change, cancel or suspend this contest should a virus, bug or other cause beyond the reasonable control of TELUS and its advertising and promotional agencies, corrupt the security or proper administration of the contest or for any other reason. Entries are subject to verification and will be declared invalid if they are illegible, mechanically reproduced, mutilated, forged, falsified, altered or tampered with or unlawful in any way. Entries submitted by unauthorised means or by spamming will be disqualified. In the event that it has been determined that an entry has been made in an manner not sanctioned by these rules and/or an entrant has submitted more than the number of entries permitted by these rules, the entrant and all of his/her entries will be disqualified. Entrants agree to abide by the contest rules and the decisions of the contest judges, which are final. This contest is subject to all applicable laws and regulations. 6. ALL DECISIONS of the contest administrators are final and the Contest sponsors and judging organization shall not be held responsible for any errors or negligence, including but not limited to any computer, online, telephone or technical malfunctions that may arise or occur in connection with the Contest. The winner must sign a standard release prior to being awarded the Prize, which release shall include confirmation of compliance with the Contest rules and regulations and acknowledgement that Contest sponsors and the contest administrators are not responsible for any claims or liabilities arising out of any matter related to the Contest and the Prize. The Contest is subject to all applicable federal, provincial and municipal laws. 7. PRIVACY The personal information that you provide in this contest may be used or disclosed by TELUS for the purposes identified in the TELUS Privacy Commitment, which is available at telus.com 8. ALL PRIZES are seen as a taxable benefit to the employee, all applicable taxes will be paid TELUS
