Thank you for participating in the NEADS (National Educational Association of Disabled Students) Graduate Experience survey. The information collected will be used to help us gather data on experiences students with disabilities have during their graduate studies at a post-secondary institution. The survey will go into detail regarding your experiences as a graduate student with a disability, which may make you find some questions difficult. At any time you are free to discontinue the survey or pause and return to complete it at a later time. If you feel you need to talk to someone regarding issues this survey may bring up, your institution's counselling centre or NEADS can be contacted to offer assistance. Be assured, the strictest confidentially will be observed. Your name or any other distinguishing information will not be published nor will your contact information be shared without your expressed permission. You will find a combination of fill in the blanks, drop down menu questions, rating scales, and check boxes.

Please answer the questions to the best of your ability. All questions refer to your CURRENT or MOST RECENT graduate program of study.

The survey should take you no more than 45 minutes to complete. There will be an opportunity at the end of the survey for additional comments. You will also be given the option of providing your contact information for follow-up informative interviews. Please note this is completely voluntary.

You can also provide contact information if you wish to be included in a compensation draw for an iPad 3 for participating in the survey. If you start the survey but decide you no longer want to participate you are able to stop and your information will not be used in the final data analysis. However, if you complete the survey and do not provide contact information and decide you no longer wish to participate it will be impossible to pull out the data you have already given.
